Please be advised that our mobile clinic is very tall, wide, and heavy. We request a detailed description of driveway conditions before we drive to your home.

I authorize the staff and doctors of Interior Mobile Vet to perform any procedures that are considered therapeutically and / or diagnostically necessary for the care of my pet, including the administration of anesthesia. If I cannot be reached, I authorize the staff to perform treatment as is necessary to preserve the life of the patient until I can be contacted for further authorization. I understand that no guarantee of successful treatment is made.*
I accept financial responsibility for the treatment(s) of the above-named patient(s) and understand that financial arrangements must be made upon release of my pet or when service is otherwise completed.
Cancelling with less than 7 days notice is subject to a $150 cancellation fee.*
We require a card on file for all new clients before scheduling.
